Sounds good, even looks good, but it's not worth $9 (Â£4.50) a month.

If you're a developer, a good host with decent bandwidth, and you having a good, reliable template for your forms (ideally in OOP PHP) and you're all set...

That took is really nice and neat. Perhaps it could spare a little bit more information on how to process entries.
Most users can design a form. Usually the difficult part is to get to know how to best collect and process data.
